Our Care Home Team comprises of Nurses, a Paramedic, Health Care Assistant and Care Co-ordinator, this team provide daily support to the 27 Nursing and Residential Homes within our area.  This includes same day visits for residents who are unwell, plus regular telephone or in person rounds at the home.  The Care Co-ordinator also calls all of the homes at least twice each week just to check in and make sure everything is OK with our residents.
