As a PCN we work very closely with other local organisations and agencies. We want to ensure our patients get joined up care and that no organisation works in isolation when supporting our patients.

Every two weeks we have a multi-disciplinary meeting that is facilitated by our MDT Co-ordinator, the purpose of these meetings is to ensure a co-ordination of care is maintained for any of our patients who have complex health and/or social care needs.

If one of our patients is referred for discussion at the MDT Meeting, this would never be done without the patients consent.
