This service is led by a Team of very experienced Advanced Nurse Practitioners.

• This service runs from 8am to 8pm Monday to Friday and 9am to 5pm every Saturday.
• Daily face to face clinics are held across practices within the PCN 
• Telephone consultations are also available every day 
• Most referrals will come into this service via e-consult with the aim to offer a same day consultation as much as possible.
• Patients may be offered an appointment at a surgery that isn’t their own registered practice, but we have the technology in place to ensure the clinician can access the full patient record to ensure a holistic assessment can take place.
• Any consultation is written directly into the patient record to ensure robust and accurate communication is in place.
